Body Mass Index (BMI) atau Indeks Massa Tubuh (IMT) adalah suatu ukuran yang digunakan untuk menentukan apakah seseorang memiliki berat badan yang sehat berdasarkan tinggi badannya. BMI dihitung dengan membagi berat badan seseorang dalam kilogram dengan kuadrat tinggi badannya dalam meter (kg/m²).

  1. Kurang Berat Badan: BMI kurang dari 18,5
  2. Berat Badan Normal: BMI antara 18,5 dan 24,9
  3. Kelebihan Berat Badan: BMI antara 25 dan 29,9

Fungsi dan Kegunaan BMI:

  • Penilaian Kesehatan: BMI digunakan sebagai indikator untuk menilai risiko kesehatan yang berkaitan dengan berat badan, seperti penyakit jantung, diabetes tipe 2, dan tekanan darah tinggi.
  • Pemantauan Kesehatan: Dokter dan ahli gizi sering menggunakan BMI untuk memantau perubahan berat badan dan memberikan rekomendasi gaya hidup sehat.

Keterbatasan BMI:

  • Tidak Mengukur Lemak Tubuh Secara Langsung: BMI tidak membedakan antara berat dari lemak, otot, dan tulang.
  • Variasi Individu: Faktor seperti usia, jenis kelamin, dan etnisitas dapat mempengaruhi interpretasi BMI. Misalnya, atlet dengan massa otot tinggi mungkin memiliki BMI yang tinggi meski kadar lemak tubuhnya rendah.
  • Tidak Menilai Distribusi Lemak: BMI tidak memberikan informasi tentang distribusi lemak tubuh, yang juga penting untuk menilai risiko kesehatan.

Kesimpulan: BMI adalah alat yang sederhana dan umum digunakan untuk menilai apakah seseorang memiliki berat badan yang sehat. Untuk melakukan interpetasi EKG, perawat perlu mengetahui anatomi dan sistem kelistrikan jantung, ditambah pengetahuan tentang mesin EKG yang digunakan baik setting mesin maupun kertas yang digunakan. Jika perawat sudah mampu membaca EKG meskipun hanya dasarnya sekalipun, maka perawat ini akan sangat bernilai bagi sebuah rumahsakit.

Anatomi dan Fisiologi Jantung

Jantung merupakan organ muskular yang terletak di ruang antara paru (mediastinum) di rongga tengah dada. Besar jantung seukuran kepalan tangan orang dewasa dan sekitar 2/3 jantung terletak di sebelah kiri garis tengah sternum. Jantung dilapisi membran yang disebut perikardium. Perikardium ini mengelilingi jantung Anda seperti sebuah kantung. Lapisan luar perikardium mengelilingi akar pembuluh darah utama jantung Anda dan dilekatkan oleh ligamen ke tulang belakang, diafragma, dan bagian lain dari tubuh Anda.

Jantung terdiri dari empat ruang yaitu atrium kiri dan kanan, ventrikel kiri dan kanan. Atrium kanan akan menerima darah dari seluruh tubuh melalui vena kava superior dan inferior, darah yang diterima atrium kanan ini memiliki kadar oksigen yang rendah. Darah yang masuk ke atrium kanan selanjutnya akan dipompa ke ventrikel kanan melewati katub trikuspid, kemudian darah ini akan dialirkan ke arteri pulmonalis menuju paru-paru. Terjadi proses pertukaran gas di alveoli yang menghasilkan darah kaya oksigen.

Darah kaya oksigen akan dialirkan ke atrium kiri melalui vena pulmonalis dan selanjutnya ke ventrikel kiri. Dari ventrikel kiri, darah akan di pompa dan dialirkan keseluruh tubuh.

Pasokan darah ke jantung

Jantung memang berfungsi untuk memompa darah ke seluruh tubuh, namun jantung juga perlu pasokan oksigen dan nutrisi untuk tetap bekerja dengan baik.

Arteri koroner

Arteri koroner terdiri dari arteri koroner kanan dan arteri koroner kiri.

Arteri koroner kiri (Left main coronary artery-LM) mempunyai dua cabang besar ramus desenden anterior kiri (Left anterior descenden – LAD) dan ramus sirkumfleks (Left circumplec – LCx). Arteri ini melingkari jantung dalam dua lekuk anatomis eksternal yaitu; pertama, sulkus atrioventrikular yang melingkari jantung antar atrium dan ventrikel. kedua, sulkus interventrikel yang memisahkan kedua ventrikel.

Arteri desendens anterior kiri(LAD) bertugas memasok darah ke bagian depan dan bawah ventrikel kiri serta bagian depan septum. Arteri sirkumfleks bertugas memasok darah ke atrium kiri dan samping serta belakang ventrikel kiri.

Arteri koroner kanan (Right coronary artery – RCA) berjalan kesisi kanan jantung pada sulkus atrioventrikurel kanan. Tugasnya memasok darah ke atrium kanan, ventrikel kanan, bagian bawah ventrikel kiri dan belakang septum.

Vena jantung

Sistem vena jantung mempunyai tiga bagian, yaitu:

  1. Vena tebesian, merupakan sistem terkecil yang menyalurkan sebagian darah dari miokardium atrium dan ventrikel kanan.
  2. Vena kardiaka anterior, mempunyai fungsi mengosongkan sebagian besar vena ventrikel ke atrium kanan.
  3. Sinus koronarius dan cabangnya merupakan sistem vena yang paling besar dan penting. Berfungsi menyalurkan pengembalian darah vena miokard ke dalam atrium kanan melalui ostium sinus koronarius yang bermuara disamping vena kava inferior.

Sistem elektrofisiologi jantung

Sistem konduksi jantung adalah sistem yang mengatur irama dan kecepatan kontraksi otot jantung. Sistem ini terdiri dari beberapa bagian yang menghasilkan dan menghantarkan sinyal listrik ke seluruh jantung. Sinyal listrik ini menyebabkan jantung berkontraksi secara teratur dan terkoordinasi, sehingga dapat memompa darah ke seluruh tubuh.

Berikut adalah bagian-bagian dari sistem konduksi jantung dan fungsinya:

Sinoatrial (SA) Node: adalah bagian yang menghasilkan sinyal listrik pertama. SA Node berada di dinding atrium kanan (ruang atas jantung) dan berfungsi sebagai pacemaker alami jantung. Sinyal listrik dari SA node merangsang atrium kanan dan kiri untuk berkontraksi, mendorong darah ke ventrikel (ruang bawah jantung). SA node mengatur ritme jantung berkisar pada 60-100 x/menit dengan mempertahankan kecepatan depolarisasi.

Atrioventrikular (AV) Node: adalah bagian yang menerima sinyal listrik dari nodus SA dan menundanya sejenak. Nodus AV berada di septum (dinding pemisah) antara atrium kanan dan kiri. Penundaan ini memungkinkan ventrikel memiliki waktu untuk mengisi darah sebelum berkontraksi serta memproteksi ventrikel dari stimulasi berlebihan atrium seperti pada atrial fibrilasi. AV Node akan menghasilkan impuls 40-60 x/menit.

Berkas (bundle) His: adalah bagian yang menghubungkan nodus AV dengan cabang-cabang berkas. Berkas His berjalan di sepanjang septum antara ventrikel kanan dan kiri.

  • Cabang-cabang berkas: adalah bagian yang membagi sinyal listrik dari berkas His ke dua arah, cabang berkas kanan mengirim impuls ke ventrikel kanan dan cabang berkas kiri mengirim impuls ke ventrikel kiri. Cabang-cabang berkas berakhir di serabut Purkinje.
  • Serabut Purkinje: Ini adalah bagian yang menyebarluaskan sinyal listrik ke seluruh miokardium (lapisan otot) ventrikel. Serabut Purkinje menyebabkan ventrikel kanan dan kiri berkontraksi secara bersamaan, memompa darah ke paru-paru dan tubuh.

Aksi potensial jantung

Aksi potensial dibagi menjadi 5 fase

Fase 0 , depolarisasi cepat: terjadi pemasukan cepat Na+ dari luar sel ke dalam sel melalui saluran Na+. Ion K+ bergerak ke luar sel dan Ca++ bergerak lambat masuk kedalam sel melalui saluran Ca++. Sel akan terdepolarisasi dan dimulailah kontraksi jantung yang ditandai dengan kompleks QRS pada EKG. Selanjutnya terjadi repolarisasi segera yang terdiri dari tiga fase

Fase 1 , Repolarisasi dini: saluran Na+ akan menutup sebagian, memperlambat aliran Na+ kedalam sel. Pada saat bersamaan, Cl masuk kedalam sel dan K+ keluar melalui saluran K+. Alhasil terjadi penurunan jumlah sel positif dalam sel yang menimbulkan gelombang defleksi negatif pada kurva aksi potensial.

Fase 2 , fase plateu: terjadi pemasukan lambat Ca++ ke dalam sel melului saluran Ca++. Ion K terus keluar dari sel melalui saluran K+. Fase ini ditandai dengan segmen ST pada EKG.

Fase 3 , repolarisasi cepat akhir: terjadi downslope aksi potensial, dimana K+ bergerak cepat keluar sel. Saluran Ca++ dan Na+ tertutup sehingga Ca++ dan Na+ tidak bisa masuk ke dalam sel. Pengeluaran cepat K+ menyebabkan suasana elektrik didalam sel menjadi negatif. Hal ini menjelaskan terjadinya gelombang T (repolarisasi ventrikel) pada EKG. Jika saluran K+ dihambat, terjadi pemanjangan aksi potensial.

Fase 4 , Resting membrane potential: kembali pada keadaan istirahat, Na+ dijumpai banyak dalam sel, serta K+ banyak diluar sel. Pompa Na+ K+ akan diaktivasi untuk mengeluarkan Na+ dan memasukkan K+ ke dalam sel. Jantung mengalami polarisasi (siap untuk stimuli berikutnya).

Elektrokardiogram dan Elektrokardiografi

Pengertian elekrokardiogram adalah grafik yang menggambarkan rekaman listrik jantung, sedangkan elektrokardiografi adalah ilmu yang mempelajari aktivitas listrik jantung. Secara sederhana dapat dipahami bahwa elektrokardiogram(EKG) adah kertas hasil perekaman EKG dan elekrokardiogrami adalah saat kita menginterpretasikan bacaan dari rekaman tersebut.

Fungsi EKG diantaranya untuk mendeteksi:

  1. Aritmia jantung
  2. Hipertrofi atrium dan ventrikel
  3. Iskemik dan infark miokard
  4. Efek obat – obatan seperti (digitalis, anti aritmia dll)
  5. Gangguan keseimbangan elektrolit khususnya kalium.
  6. Penilaian fungsi pacu jantung.

Yaitu merekam perbedaan potensial dari dua elektroda, sadapan ini ditandai dengan angka romawi I, II, dan III

  • Sadapan I untuk merekam bedapotensial antara tangan kanan (RA) dan tangan kiri (LA) dimana tangan kanan bermuatan (-) dan tangan kiri (+).
  • Sadapan II untuk merekam beda potensial antara tangan kanan (RA) dengan kaki kiri (LF), dimana kaki tangan kanan bermuatan (-) dan kaki kiri bermuatan(+).
  • Sadapan III untuk merekam beda potensial antara tangan kiri (LA) dengan kaki kiri (LF), dimana tangan kiri bermuatan (-) dan kaki kiri bermuatan (+).

Sadapan Unipolar

Sadapan unipolar terbagi menjadi sadapan unipolar ekstremitas dan sadapan unipolar prekordial.

Sadapan unipolar ekstremitas

Sadapan unipolar ekstremitas untuk merekam besar potensial listrik pada satu ekstremitas, elektroda eksplorasi diletakkan pada ekstremitas yang mau diukur. Gabungan elektroda-elektrodapada ekstremitas yang lain membentuk elektroda indiferen (potensial 0).

Sadapan aVR , merekam potensial listrik pada tangan kanan (RA) dimana tangan kanan bermuatan (+), tangan kiri dan kaki kiri membentuk elektroda indiferen.

Sadapan aVL , merekam potensial listrik pada tangan kiri (LA) dimana tangan kiri bermuatan (+), tangan kanan dan kaki kiri membentuk elektroda indiferen.

Sadapan aVF , merekam potensial listrik pada kaki kiri (LF) dimana kaki kiri bermuatan (+) tangan kanan dan tangan kiri membentuk elektroda indiferen.

Sadapan unipolar prekordial

Sadapan unipolar prekordial merekam besarpotensial listrik jantung dengan bantuan elektroda eksplorasi yang ditempatkan di beberapa dinding dada. Elektroda indiferen diperoleh dengan menggabungkan ketiga elektroda ekstremitas. Letak sadapan unipolar prekordial:

V1 di ruang interkostal IV garis sternal kanan

V2 di ruang interkostal IV garis sternal kiri

V3 di pertengahan antara V2 dan V4

V4 di ruang interkostal V garis midclavikula kiri

V5 sejajar V4 di garis aksila depan

V6 sejajar V5 di garis aksila tengah

Umumnya perekaman EKG lengkap dibuat 12 sadapan (lead), akan tetapi pada keadaan tertentu perekaman dibuat sampai dengan V7,V8, dan V9 atau V3R dan V4R.

Kertas EKG

Kertas EKG adalah kertas grafik yang merupakan garis horizontal dan garis vertikal dengan jarak 1mm = 1 kotak kecil (KK). Garis yang lebih tebal terdapat pada setiap 5mm = 5 KK = 1 kotak besar (KB).

Garis horizontal menunjukkan waktu, dimana 1 kotak kecil (KK) = 0,04 detik, ini didapatkan karena standar perekaman EKG adalah 25 mm/detik dan 1 KK = 1 mm.

Jadi 1 KK = 1 mm = 0,04 detik

sehingga jika 5 KK menjadi = 0,20 detik = 1 kotak besar (KB)

25 kotak kecil (KK) = 1 detik = 5 Kotak besar(KB)

150 kotak kecil (KK) = 6 detik = 20 Kotak besar(KB)

1500 kotak kecil (KK) = 60 detik = 300 Kotak besar(KB)

Garis vertikal menggambarkan voltage, diman 1 kotak kecil (KK) = 0,1 mv. Nilai 0,1 didapatkan karena standar perekaman EKG menggunakan kekuatan atau lebih dikenal dengan kalibrasi 1 dengan tanda garis defleksi positif 10 mm atau 10 kotak kecil atau 2 kotak besar.

Kurva EKG

Kurva EKG menggambarkan proses listrik yang terjadi pada atrium dan ventrikel. EKG normal terdiri dari gelombang P,Q,R,S dan T serta kadang terlihat gelombang U.

Gelombang P

Gelombang P merupakan gambaran proses depolarisasi atrium. Gelombang P yang normal mempunyai ciri:

  • lebar < 0,12 detik dan selalu positif di Lead I
  • tinggi < 0,3 mv dan selalu negatif di Lead aVR

Gelombang QRS

Gelombang QRS merupakan gambaran depolarisasi ventrikel, gelombang QRS yang normal mempunyai ciri:

  • lebar 0,06 – 0,12 detik
  • untuk tinggi nya tergantung Lead

Gelombang Q

Gelombang Q adalah defleksi negatif pertama pada gelombang QRS, normalnya:

  • lebar 0,04 detik
  • tinggi/ dalamnya < ⅓ tinggi R

Gelombang R

Gelombang R adalah defleksi positif pertama pada gelombang QRS, umumnya positif di Lead I, II, V5 dan V6. Sedangkan di Lead aVR, V1, V2 biasanya kecil atau tidak ada.

Gelombang S

Adalah defleksi negatif setelah gelombang R, di Lead aVR dan V1 gelombang S terlihat dalam. Dari V2 ke V6 gelombang S akan terlihat makin lama makin menghilang.

Gelombang T

Gelombang T merupakan proses repolarisasi ventrikel, gelombang T positif di Lead I,II, V3 sampai V6 dan terbalik di aVR.

Gelombang U

Adalah gelombang yang timbul sebelum gelombang T dan sesudah gelombang P berikutnya. Penyebab timbulnya gelombang U masih belum diketahui namun diduga akibat repolarisasi lambat sistem konduksi intraventrikel.

Interval PR

Interval PR dihitung dari permulaan gelombang P sampai permulaan gelombang QRS. nilai normal berkisar antara 0,12 s.d. 0,20 detik. Ini merupakan waktu yang dibutuhkan untuk depolarisasi atrium dan jalannya impuls melalui berkas His sampai permulaan depolarisasi ventrikel.

Segmen ST

Segmen ST diukur dari akhir gelombang S sampai awal gelombang T, Segmen ini normalnya isoelektris tetapi pada lead prekordial bervariasi antara -0,5 sampai +2mm

Cara Interpretasi EKG

Cara menilai EKG lengkap:

  1. Tentukan apakah denyut jantung berirama teratur atau tidak.
  2. Tentukan berapa frekuensi jantung (HR).
  3. Tentukan gelombang P apakah normal atau tidak.
  4. Tentukan irama PR normal atau tidak.
  5. Tentukan gelombang QRS normal atau tidak.
  6. Interpretasi.

Menentukan sumbu aksis jantung

Tentukanlah jumlah dari defleksi pada masing-masing sadapan (gelombang R dikurangi gelombang S) kemudian ilustrasikan sebagai vektor pada masing-masing sumbu. Dari kedua vektor ini dapat dibuat resultan yang menggambarkan sumbu QRS.

Tes ini sangat penting untuk menilai fungsi paru-paru, keseimbangan asam basa tubuh, dan status metabolisme.

Alasan penghambilan gas darah:

  • Menilai fungsi paru-paru: Mengukur seberapa baik paru-paru menyerap oksigen dan mengeluarkan karbon dioksida.
  • Mengetahui keseimbangan asam basa: Menentukan apakah tubuh terlalu asam (asidosis) atau terlalu basa (alkalosis).
  • Memantau penyakit paru kronis: Seperti asma, COPD, atau fibrosis kistik.
  • Mengevaluasi efek pengobatan: Misalnya, pada pasien yang menggunakan ventilator atau terapi oksigen.
  • Mendiagnosis gangguan metabolisme: Seperti diabetes, gagal ginjal, atau gangguan elektrolit.

Parameter Utama dalam AGD

  • pH: Menunjukkan tingkat keasaman atau kebasaan darah. Nilai normal: 7,35-7,45.
  • PaO2: Tekanan parsial oksigen dalam darah arteri. Menunjukkan kadar oksigen terlarut dalam darah. Nilai normal: 80-100 mmHg.
  • PaCO2: Tekanan parsial karbon dioksida dalam darah arteri. Menunjukkan kadar karbon dioksida terlarut dalam darah. Nilai normal: 35-45 mmHg.
  • HCO3-: Bikarbonat. Ion bikarbonat berperan penting dalam mempertahankan keseimbangan asam basa. Nilai normal: 22-26 mEq/L.
  • SatO2: Saturasi oksigen. Persentase hemoglobin yang terikat oksigen. Nilai normal: 95-100%.

Cara menentukan hasil AGD

Pertama lihat angka pH apakah diatas atau dibawah range normal. Untuk hasil AGD dengan pH dibawah 7,35 disebut Asidosis sedangkan pH diatas 7,45 disebut alkalosis.

Kedua lihat hasil PCo2 dan HCo3 apakah mengalami kenaikan atau penurunan dari range normal. Nilai PCo2 27 (menurun) dan HCo3 15 (menurun).

Interpretasi AGD

Ketiga, dapat kita simpulkan hasil AGD tersebut dengan pH ⬇ , pCO2 ⬇, HCO3 ⬇ merupakan Asidosis Metabolik. Interpretasi asidosis metabolik ini sudah cukup yang baru belajar membaca AGD, walaupun mungkin bila temen sejawat sedang kuliah dan ditanya sama dosennya; ini asidosis metaboliknya terkompensasi atau tidak? Jangan bingung langsung saja cek hasil BE nya dan cocokkan ke tabel diatas. Ternyata dari hasil BE -13,7 ⬇, kita simpulkan bahwa ini adalah gangguan asidosis metabolik terkompensasi sebagian.

Asidosis Metabolik

Merupakan kondisi di mana tubuh menghasilkan terlalu banyak asam atau gagal mengeluarkan cukup asam.

Penyebab: Diabetes ketoasidosis, Gagal ginjal kronis, Diare berat, Penggunaan obat-obatan tertentu (misalnya, aspirin dalam dosis tinggi), Keracunan metanol atau etilen glikol.

Gejala: Kelelahan, Mual dan muntah, Nafas dalam dan cepat, Sakit kepala, Kebingungan, Koma (dalam kasus yang parah).

Asidosis Respiratorik

Merupakan kondisi di mana paru-paru tidak mampu mengeluarkan karbon dioksida dengan efisien.

Penyebab:Penyakit paru obstruktif kronik (PPOK), Asma berat, Pneumonia, Cedera dada, Penggunaan obat penenang atau sedatif.

Gejala: Sesak napas, Sakit kepala, Kelelahan, Kebingungan, Kulit kemerahan.

Alkalosis Metabolik

Merupakan kondisi di mana tubuh kehilangan terlalu banyak asam atau memiliki terlalu banyak basa.

Penyebab: Muntah berlebih, Penggunaan diuretik yang berlebihan, Penggunaan antasid berlebihan, Keracunan kortikosteroid.

Merupakan kondisi di mana pernapasan terlalu cepat dan dalam, sehingga terlalu banyak karbon dioksida yang dikeluarkan.

Penyebab: Hiperventilasi (bernapas terlalu cepat) akibat kecemasan, nyeri, atau penyakit paru tertentu, Demam tinggi, Sepsis.

Nyeri adalah rasa pengalaman sensorik dan emosional yang tidak menyenangkan terkait kerusakan aktual atau potensial.

(Gélinas et al., 2004)

Tindakan medis baik ringan maupun berat yang ditujukan untuk penyelamatan dan membantu pasien bertahan hidup sering kali menyebabkan keluhan nyeri serta ketidaknyamanan pada pasien. Nyeri merupakan respon subjektif dari pasien yang berbeda- beda dari individu satu dengan individu lainnya. Tindakan ringan mungkin tidak berdampak nyeri pada satu individu namun pada individu lain dapat melaporkan adanya nyeri.

Respon nyeri timbul berdasarkan pengalaman sensori pasien dan tidak dapat disama ratakan untuk semua individu. Faktor usia, jenis kelamin menentukan tingkat respon nyeri yang timbul.

Adanya nyeri yang dirasakan oleh pasien yang berlangsung sebentar ataupun terus menerus dapat mempengaruhi hemodinamik pasien bahkan dapat menyebabkan syok neurologik. Penilaian nyeri secara kontinyu sangat penting agar dapat menghindari dampak buruk dari nyeri yang dirasakan pasien.

Mekanisme Terjadinya Nyeri

Terjadi pelepasan mediator inflamasi setelah cedera jaringan. Mediator merangsang nosiseptor, yang mentransmisikan impuls nyeri ke kornu dorsalis medula spinalis. impuls nyeri tiba di korteks somatosensori, di mana lokalisasi dan makna nyeri terjadi .

Asesment Nyeri di Intensive/Critical Unit

Pasien pasien yang berada di ruang Intensif adalah pasien yang paling banyak mendapat tindakan medis. Pemasangan akses intravena, selang nasogastrik, kateter uretra, suction, dan intubasi endotrakeal sering dikerjakan pada pasien di ruang intensif. Tingkat nyeri yang dirasakan oleh pasien di ruang intensif menjadi lebih tinggi dan dapat berlangsung kontinyu. Terdapat 71% pasien di ruang intensif merasakan rasa nyeri terkait prosedur tindakan medis.

Klasifikasi nyeri:

  1. Berdasarkan jenis nyeri; nyeri somatik, nyeri viseral, dan nyeri psikologenik
  2. Berdasarkan lama timbulnya; nyeri akut dan kronik.
  3. Berdasarkan penyebab; nyeri onkologik dan nyeri non onkologik.
  4. Berdasarkan derajat nyeri; nyeri ringan, sedang, dan berat.

Metode pengkajian :

  1. Pengkajian nyeri secara verbal
  2. Pengkajian nyeri secara non verbal

Nyeri dapat menimbulkan respon fisiologis seperti peningkatan denyut jantung, pernafasan dan tekanan darah. Namun respon fisiologis tersebut tidak dapat digunakan sebagai penilaian terjadinya nyeri karena pasien- pasien sakit kritis yang mengalami syok atau sepsis juga dapat menimbulkan respon yang sama .

Pengkajian nyeri secara verbal adalah pengumpulan data respon pasien terhadap nyeri yang diungkapkan langsung secara verbal oleh pasien. Pasien dapat mengungkapkan intensitas, kekuatan dan frekuensi nyeri yang dialami. Sedangkan pengkajian nyeri secara non verbal yaitu pengumpulan data nyeri pasien melalui ekspresi wajah dan atau gerakan motorik pasien.

Pada pasien yang terpasang ventilator, mendapat sedasi, atau mengalami penurunan kesadaran jelas tidak dapat mengungkapkan rasa nyeri yang dialami. Sebagai contoh ketika perawat memasang NGT, meskipun pasien mengalami penurunan kesadaran dan tidak dapat berbicara namun pasien saat dipasang NGT akan menyeringai dan mungkin menggoyangkan kepala.

Ruang ICU didominasi oleh pasien yang mengalami penurunan kesadaran dan memerlukan terapi invasif untuk bertahan hidup. Pasien- pasien seperti itu tidak dapat mengungkapkan secara verbal nyeri yang dialami. Dibutuhkan sebuah instrumen yang dapat menilai intensitas nyeri dari ekspresi wajah dan gerakan motorik yang dilakukan oleh pasien saat mengalami nyeri.

Critical-care pain observational tool (CPOT)

Menilai ekspresi wajah pasien, gerakan motorik, tarikan otot-otot, serta komplian ventilator. Instrumen ini dapat diterapkan pada pasien dengan ventilator maupun pasien tanpa ventilator.

CPOT dan BPS menjadi instrumen penilaian nyeri non verbal yang paling dapat diandalkan pada pasien-pasien sakit kritis. Bahkan penilaian kedua instrumen tersebut memiliki kesesuaian (agreement) sebesar 0,937 saat istirahat dan 0,265 saat positioning yang diukur menggunakan rumus Kappa.

Cara mengetahui perlu tidaknya terapi oksigen

Secara sederhana apabila seseorang nampak sesak nafas, mengembangkan dada serta nafas dengan cepat maka bisa dipastikan memerlukan oksigen tambahan. Namun sejak adanya pandemi covid 19 yang menyebabkan penderitanya mengalami happy hipoksia, diperlukan alat ukur kebutuhan oksigen yang lebih objektif. Oksimetri dapat membantu mengetahui kadar oksigen seseorang, alat ini mudah digunakan bagi orang non medis. Di rumah sakit penggunaan oksimetri sangat membatu mempercepat tindakan medis sebelum hasil analisa gas darah lengkap pasien keluar.

Tujuan Terapi Oksigen

  1. Meningkatkan kandungan oksigen dalam darah untuk mencukupi kebutuhan metabolisme aerob dengan target PaO2 >60mmHg, SaO2 94 – 98%, sedangkan pada pasien hipercapnic target SaO2 88 – 92%.
  2. Mengatasi hipoksemia.
  3. Menurunkan kerja pernafasan.
  4. Menurunkan kerja jantung.

Secara umum tujuan terapi oksigen adalah untuk mengoptimalkan oksigenasi jaringan dan meminimalkan asidosis respiratorik.

Terapi Oksigen pada Keadaan Hipoksia

Pada orang dewasa laki-laki membutuhkan kurang lebih 225-250ml oksigen permenit, bila selama 4-6 menit seseorang berhenti bernafas maka akan mengganggu aliran oksigen ke dalam tubuh dan jaringan.

Untuk manifestasi klinik hipoksia cukup bervariasi dan tidak spesifik tergantung pada lamanya hipoksia (akut atau kronik) Manifestasi klinik dapat berupa perubahan status mental/ sikap labil, pusing, dispneu, takipneu, respiratory distress, dan aritmia. Sianosis bisa dianggap sebagai kondisi hipoksia asalkan penderita tidak mengalami anemia. Oksimetri bisa digunakan untuk mengetahui kondisi hipoksia, bila saturasi kurang dari 90% diperkirakan hipoksia dan membutuhkan oksigen.

Selain oksimetri, pemeriksaan laboratorium perlu dilakukan untuk menentukan hipoksia. Pemeriksaan analisa gas darah dapat memberi gambaran PaO2 , PCO2 serta parameter lainnya.

Dalam pemmberian oksigen harus dipertimbangkan apakah pasien benar-benar membutuhkan oksigen, apakah dibutuhkan oksigen jangka pendek atau terapi oksigen jangka panjang. Pemberian oksigen harus diatur dalam jumlah yang tepat dan dievaluasi agar mendapat manfaat terapi dan menghindari toksisitas.

HFNO merupakan singkatan dari High Flow Nasal Oxygen atau secara sederhana; terapi oksigen aliran tinggi yang diberikan melalui hidung. Beberapa orang menyebut High Flow Nasal Canulla (HFNC) karena terapi ini diberikan hanya melalui selang kanul oksigen. Srategi pengobatan dengan HFNO telah ada sejak beberapa tahun sebelumnya, namun baru dikenal dan digunakan secara luas sebagai pengobatan saat pandemi covid 19. Penggunaan HFNO sangat efektif pada pasien dengan klinis menderita coronavirus; kesadaran composmentis namun mengalami gagal nafas hipoksia akut (happy hipoksia).

Sistem Kerja HFNO

HFNO bekerja dengan prinsip bernoulli dimana tekanan terendah mempunyai kecepatan aliran tertinggi. Alat seperti HFNO dapat mengalirkan udara dan fraksi oksigen secara konstan. HFNO dapat mengalirkan udara hingga 60ml/menit dengan fraksi oksigen 21% – 100%.

Manfaat HFNO

  1. Kecepatan aliran udara sesuai dengan kecepatan inspirasi pasien.
  2. Dapat mengurangi deadspace.
  3. Meningkatkan pertukaran CO2
  4. Aliran udara dihangatkan melalui humideifier sehingga memberikan kelembaban dan kenyamanan pada pasien.
  5. Menghindari Intubasi pada pasien sakit ringan – sedang.
  6. Dapat digunakan dirumah dengan biaya yang lebih ringan daripada ventilator invasif.

Kekurangan HFNO

  1. Hanya dapat digunakan pada pasien yang masih sadar.
  2. Hanya dapat membuat PEEP yang rendah.
  3. Penggunaaan pada pasien kritis berat mungkin dapat membuat keterlambatan intubasi.
  4. Biaya perawatan lebih tinggi dan memperlama longstay di rumah sakit. Studi dengan hasil yang berbeda dapat anda baca dari artikel BMC oleh Mellado-Artigas et al.
  5. Berisiko terjadi aerolisasi.

HFNO dapat menurunkan kebutuhan intubasi sebesar 4,4%, namun tidak menurunkan resiko kematian pada pasien dengan gagal napas hipoksemia akut.

Cara Penggunaan HFNO

Setiap produk memiliki karakteristik dan user interface yang berbeda, namun pada dasarnya proses pemakaiaannya hampir sama. Pertama, sambungkan port oksigen eksternal ke HFNO. Kedua atur kecepatan aliran yang diperlukan maksimal 60ml/ menit. Ketiga atur fraksi oksigen yang dibutuhkan. Keempat atur suhu humidifier.

Indikasi Penggunaan HFNO

  1. Gagal nafas akut hipoksia.
  2. Pre intubasi sebelum tindakan anaestesi di OK.
  3. Post intubasi setelah tindakan anaestesi di OK.

Kontra Indikasi HFNO

  1. Pasien gelisah, GCS dibawah 8
  2. Obstruksi jalan nafas atas
  3. Pasien mengalami epitaksis (mimisan), basal skull fractures.
  4. Gagal nafas hiperkapnia yang menyebabkan respiratory drive menurun, V/Q Mismatch.

HFNO untuk pasien covid 19 ringan sedang

HFNO mungkin disediakan di Rumah Sakit rujukan covid 19 namun dengan jumlah yang sangat terbatas. Mungkin banyak pasien covid 19 yang harus mengantre untuk mendapatkan terapi ini, sehingga kebanyakan hanya diberikan oksigen melalui masker. Pasien penderita covid 19 pembiayaannya telah ditanggung oleh pemerintah, termasuk penggunaan alat HFNO.

Masalah kemudian muncul saat pasien covid 19 dinyatakan sembuh dari virus ini. Secara klinis, pasien mungkin masih sesak nafas, ataupun masih terdapat keluhan lainnya, hal ini umum terjadi pada beberapa penyintas covid 19. Dampak kerusakan paru dan metabolik masih tetap ada walaupun virus corona telah hilang. Happy Hipoksia yang masih terjadi tetap membutuhkan perawatan. Pembiayaan pasien berganti dari ditanggung pemerintah menjadi di tanggung pribadi atau asuransi kesehatan dengan limit pembiayaan tertentu.

Dengan biaya operasional yang cukup tinggi akan menguras uang cukup banyak. Anda mungkin perlu strategi lain dalam menghadapai limit pembiayaan, seperti keluar dari rumah sakit untuk homecare dan menyewa HFNO sendiri untuk digunakan di rumah. Kebanyakan, pasien covid 19 yang mengalami happy hipoksia atau bahkan ARDS tetap membutuhkan terapi oksigen HFNO 1-2 minggu setelah swab PCR negatif.

Target oksigenasi pada pasien Covid 19 adalah SpO2 antara 92 – 96%. Pasien dengan hipoksia disebabkan coronavirus tidak akan mencapai target spO2 92% tanpa adanya oksigen tambahan. Pada sakit ringan mungkin cukup diberikan oksigen nasal atau simple mask, namun pada sakit sedang tidak cukup hanya dengan sungkup rebreathing saja. Adanya kerusakan pada jaringan paru (radang subpleural, permeabilitas vaskular meningkat,odema interstisial) perlu terapi ventilasi positif seperti HFNO ataupun ventilasi mekanik.

Pasien dengan kesadaran composmentis dan tarikan nafas yang masih adekuat cukup diberikan HFNO. Kinerja pernafasan mungkin meningkat seiring memburuknya peradangan dan meningkatnya metabolisme. Kekuatan pasien untuk bernafas dengan adekuat akan mencapai batasnya sehinggga memperburuk kondisi dan mengakibatkan peningkatan metabolik yang tidak terkompensasi. HFNO di saat seperti itu mungkin tidak lagi memberikan manfaat bagi pasien, dan disarankan untuk sesegera dilakukan intubasi endotrakeal.

Merupakan tindakan penyelamatan pasien yang mengalami sumbatan jalan nafas. Tergantung pada kondisi seseorang, pemasangan kanul dapat berlangsung secara sementara dan permanen (Puspita & Maskoen, 2019).

Trakeostomi memiliki sejarah panjang dari tahun 3600 SM di masa Mesir kuno, hingga sampai tahun 1960an telah memiliki landasan kuat dalam praktek kedokteran. Saat itu telah menjadi tindakan penyelamatan bukan hanya pada kasus sumbatan jalan nafas saja, namun juga pada kasus trauma kecelakaan dan kasus koma setelah operasi bedah saraf.

Indikasi Trakeostomi

  1. Akses jalan nafas darurat
  2. Akses jalan nafas untuk kasus prolonged ventilator
  3. Sumbatan jalan nafas atas (rongga mulut – laring)
  4. Perlindungan saluran nafas dari resiko aspirasi
  5. Retensi sputum di bronkotrakea.

Khusus pada pasien dengan prolonged ventilator, tindakan trakeostomi akan dipertimbangkan. Waktu pengerjaan biasanya tergantung klinis dan keadaan pasien. Ada dua timing yang populer, yaitu early antara hari kedua sampai sepuluh ventilator, dan late bila tindakan ini dikerjakan setelah 10 hari lebih penggunaan ventilator.

Manfaat Trakeostomi

  1. Mengurangi ruang rugi dead space
  2. Mempermudah penghisapan lendir (suction)
  3. Stabilisasi jalan nafas
  4. Mempermudah weaning (penyapihan) pasien dengan ventilator.

Teknik Trakeostomi

Teknik bedah (Surgical)

Dilakukan di kamar operasi steril dengan cara membuat sayatan horizontal sepanjang 2 cm. Otot tali dan fasia superfisial ditarik ke samping untuk mengekspos kartilago krikoid dan kelenjar tiroid. Tiroid ditarik ke superior atau isthmus diikat tergantung pada anatomi individu. Sayatan horizontal dibuat di antara cincin trakea kedua dan ketiga dan flap trakea berbasis inferior (sayatan berbentuk H terbalik) dijahit ke kulit (flap Björk).

Puntung dari dua cincin yang terbagi ditarik dengan dua klem dan kanul dengan ukuran yang sesuai dimasukkan ke dalam trakea. Otot tali ditarik kembali dengan satu atau dua jahitan dan kulit dijahit secara longgar dengan satu jahitan di kedua sisi kanul. Tabung endotrakeal dilepas, dan kanul dimasukkan dengan visualisasi langsung. Balon dikembangkan kemudian di auskultasi bidang paru untuk memastikan kanul telah berhasil di pasang (Oggiano et al., 2014).

Teknik Perkutan Dilatational Tracheostomy (PDT)

Pasien diposisikan dengan leher ekstensi dan ventilasi dengan FiO2 100%. Tabung endotrakeal ditarik sampai ujungnya setinggi laring subglotis. Krikoid dan cincin trakea pertama diidentifikasi dengan palpasi. Sayatan kulit berukuran 1 – 1,5 cm dibuat dan jarum pengenal kateter dimasukkan ke dalam lumen trakea antara cincin pertama dan kedua (Oggiano et al., 2014).

Perbedaan antara teknik PDT dan Surgical Thracheostomy

Studi berbeda dihasilkan oleh Oggiano et al., yang dipublikasikan dalam jurnal Pulmonologie tahun 2014. Trakeostomi dengan teknik surgical yang dilakukan oleh dokter spesialis berpengalaman dengan jabatan kepala departemen atau setingkatnya memiliki keunggulan dari sisi kejadian komplikasi infeksi, perdarahan intra dan pasca operasi, lama rawat di rumah sakit, weaning ventilator serta proses dekanulasi. Studi tersebut dibandingkan dengan teknik dilatasi perkutan yang dapat dikerjakan oleh dokter pasca sarjana.

Persiapan Tindakan Trakeostomi

Trakeostomi Dilatasi Perkutan sebagai tindakan pengelolaan jalan nafas memiliki resiko morbiditas dan mortalitas yang cukup tinggi sehingga harus dilakukan dengan prinsip 4 aman, yaitu: Aman tim pelaksana, Aman pasien, Aman alat, dan Aman obat (Purwaamidjaja & Maskoen, 2019).

Tim TDP terdiri dari:

  • Satu orang operator
  • Satu orang asisten (asisten 1) berada di atas kepala pasien untuk mengelola jalan nafas, suction, menarik sedikit demi sedikit Ett, dan melakukan oral higiene setelah selesai tindakan PDT
  • Satu orang asisten (asisten 2) untuk membantu pengerjaan trakeostomi; seperti kontrol perdarahan dep dengan kassa steril bila ada perdarahan, setelah wire di masukkan tangan kiri pegang ujung wire, tangan kanan pegang kassa steril dan cubit bagian insersi agar tidak keluar darah.

Aman pasien meliputi inform consent yang telah diberikan kepada keluarga pasien terkait manfaat dan komplikasi yang mungkin terjadi. Mengetahui kondisi penyakit dasar pasien untuk menentukan indikasi dan kontraindikasi TDP, mengetahui anatomi dasar pasien, serta waktu pelaksanaan tindakan pada beberapa pasien yang mungkin berbeda.

Aman alat untuk tindakan TDP meliputi:

  • Set bedah minor ditambah perlengkapan gaun operasi, lampu kepala, sarung tangan steril, masker, jelly (xylocain jell), silk 2.0 , abocath no 14 dan spuit 5cc.
  • Set trakeostomi dilatasional perkutan; dilator, wire, jarum insersi.
  • Set pengelolaan jalan nafas; laringoskop, endotracheal tube, spuit, ambubag (BVM), suction, stetoskop.
  • Alat penunjang; ventilator, bronkoskopi, monitor, USG.

Aman obat meliputi

  • Obat emergensi dipersiapkan untuk mengatasi kegawatdaruratan yang mungkin timbul; epinephrine, sulfas atropin, vasopressor.
  • Obat sedasi, analgesik, dan pelumpuh otot.

 

Trakeostomi Dilatasional Pada Pasien Sakit Kritis

Hampir sebagian besar pasien sakit kritis yang masuk ke ICU mengalami gagal nafas dan membutuhkan pengelolaan jalan nafas berupa terapi ventilasi mekanis. Tindakan intubasi endotrakeal sebagai pilihan pertama pasien-pasien yang mengalami sindrome pernafasan tersebut. Masalah kemudian muncul ketika beberapa pasien dengan ventilasi mekanis gagal weaning ataupun pada pasien yang berhasil ekstubasi kemudian mengalami sekuel kedua gagal nafas.

Pasien- pasien dengan penggunaan ventilator jangka panjang melalui selang endotrakeal beresiko mengalami edema laring, ulserasi mukosa, meningkatnya kejadian Ventilator Associated Pneumonia (VAP), serta menambah lama rawat dan biaya perawatan. Konversi dari intubasi ke trakeostomi akan memberikan manfaat pada pasien seperti meningkatkan kenyamanan, kebersihan mulut dan sekret yang mudah dikeluarkan, menurunkan resiko VAP, mengurangi penggunaan sedasi, meningkatkan prosentase keberhasilan weaning ventilasi mekanis, serta mengurangi lama rawat di ICU (Lestari & Pratiwi, n.d.).

Terdapat 10 – 24 % pasien di ICU membutuhkan trakeostomi untuk bantuan pernafasan dan weaning yang berkepanjangan. Trakeostomi teknik surgical paling sering di kerjakan di kamar operasi, hingga tahun 1985 Ciaglia et al. memperkenalkan teknik perkutan yang dapat dilakukan secara bedside. Teknik perkutan dikerjakan secara nyaman di tempat tidur pasien tanpa perlu peralatan dan pencahayaan seperti ruang operasi. Teknik perkutan juga dapat menghindari perpindahan pasien sakit kritis ke ruang operasi bila dikerjakan dengan teknik surgical. Sejak teknik perkutan menjadi populer diberbagai negara untuk kebutuhan trakeostomi pasien- pasien di ICU, trakeostomi surgical dilakukan pada kasus kasus sulit bila teknik perkutan merupakan kontraindikasi (Mallick & Bodenham, 2010).

Perawatan Pasca Trakeostomi

Pasien- pasien di ICU dengan treakeostomi yang masih membutuhkan bantuan ventilasi mekanis harus di tangani secara baik dan tepat oleh tim perawatan yang mempunyai pengalaman terlibat dalam tindakan trakeostomi, mengetahui jenis dan ukuran kanul, serta mengetahui fisiologi dan anatomi dasar pernafasan.

Tujuan perawatan kanul trakeostomi

  1. Untuk mencegah sumbatan pipa trakeostomi (plugging)
  2. Untuk mencegah infeksi.
  3. Meningkatkan fungsi pernafasan (ventilasi dan oksigenasi)
  4. Brokial toilet yang efektif.
  5. Mencegah pipa tercabut.

7 langkah perawatan kanul trakeostomi

  1. Pengelolaan kanul dan balon trakeostomi >> Untuk kanul dalam dicuci dan dibersihkan tiap 8 jam atau pershift dan lebih sering dibersihkan jika kotor karena produksi slem kental dan banyak. Jika pasien masih menggunakan ventilator tekanan balon dipertahankan antara 20-25 cmH2O. Balon kanul dapat dikempiskan pada pasien yang sudah tidak menggunakan ventilator.
  2. Penghisapan lendir/ suctioning secara berkala >> Kateter suction dimasukkan ± 8-10 cm selama 10 detik. Tindakan suction dilakukan dengan seefektif mungkin dan hindari terlalu sering suction karena dapat menyebabkan terjadi luka, perdarahan, dan infeksi pada mukosa trakeobronkial.
  3. Perawatan luka stoma >> jaga stoma tetap kering dan bersihkan sehari sekali atau jika lembab dan kotor. Bersihkan tepi luka menggunakan kassa steril dengan cairan NaCL 0,9% atau dengan kassa betadhine. Sedangkan untuk jahitan kanul trakeostomi dibuka pada hari ketujuh.
  4. Perawatan tali kanul trakeostomi >> Tujuan diikat menggunakan tali ini agar kanul trakeostomi tidak keluar sendiri dari tempatnya, atau terlepas secara tidak sengaja seperti pada kondisi batuk ataupun saat pasien tidur dan membuat gerakan yang mengakibatkan kanul terlepas. Menjaga agar tali tidak mudah terlepas, ganti tali ikat kanul tiap 3-7hari atau jika kotor dan basah. Ikatan kanul tidak boleh terlalu kencang, berikan ruang 1-2 jari agar tidak terlalu kencang.
  5. Latihan nafas/ batuk dan bicara >> latihan nafas dengan ventilasi mekanis, latihan manual (tanpa alat bantu, menutup dengan jari), latihan dengan katub atau penutup standar TDP, latihan menggunakan speaking Valve.
  6. Kontrol >> kontrol pasien terpasang trakeostomi dilakukan 1-3 bulan setelah pemasangan. Dicatat tanggal pemasangan, nomor kanul, pada saat kontrol dievaluasi apakah pasien dapat dilakukan dekanul atau harus rekanul. Bila nafas dan reflek batuk pasien telah baik, maka dapat dilakukan dekanulasi. Selama dekanulasi diobservasi pola nafas pasien apakah terjadi distorsi nafas, bila terjadi distorsi nafas maka dikerjakan dekanulasi dengan nomor kanul yang lebih kecil dari sebelumnya. Yang dinilai apakah pasien dapat dekanul atau tidak adalah reflek batuknya, walaupun masih ada produksi sekret sedikit.
  7. Edukasi >> Edukasi kepasien dan keluarganya dapat menggunakan leaflet meliputi perencanaan pasien homecare perawatan kanul, latihan nafas, pemenuhan diit dan gizi pasien, serta tatacara kontrol untuk pasien dengan terpasang kanul trakeostomi.

Dekanulasi setelah evaluasi batuk efektif yang baik, kemampuan bernafas yang adekuat. Dekanulasi harus segera dikerjakan untuk mengembalikan fungsi fisiologis pasien agar kembali normal. Keseluruhan proses perawatan pasca trakeostomi sampai dekanulasi dipimpin oleh seorang intensivist sebagai kepala tim perawatan.

Persiapan homecare pasien dengan kanul trakeostomi

Langkah ketujuh perawatan trakeostomi adalah edukasi. Inti dari edukasi ini adalah mempersiapkan keluarga agar SIAP dan berani merawat keluarganya yang terpasang kanul trakeostomi saat dirumah. Perawat melatih keluarga untuk bisa mencuci kanul dalam dengan benar, mengganti tali kanul, membersihkan stoma, cara melatih nafas pasien dengan benar, cara suction yang benar dan aman.

Keluarga penerima edukasi adalah mereka yang tinggal serumah dengan pasien atau keluarga yang bersedia menjaga dan merawat pasien saat dirumah. Pertama perawat melakukan edukasi persiapan alat yang diperlukan saat dirumah antara lain; tensi otomatis, termometer, saturasi oksigen portabel, tabung oksigen. Edukasi keluarga untuk menulis dibuku hasil tensi, suhu dan saturasi pasien setiap hari serta kebutuhan oksigen pasien.

Ajarkan keluarga untuk mencuci kanul dalam, kemudian praktik langsung didampingi perawat mulai dari memakai handscoen, mencopot kanul dalam, mencuci kanul dengan NaCL atau air hangat, memasang kembali kanul dalam ke pasien.

Peragakan di depan keluarga cara suction yang benar dan aman saat dirumah. Kemudian keluarga diminta praktik langsung mulai dari cara menghidupkan dan mematikan mesin suction, cara memegang selang suction yang benar, perkiraan kedalaman suction (7-10 cm).

Materi edukasi persiapan homecare ini dapat mencakup; cara mencuci kanul, cara suction yang benar dan aman bagi keluarga, cara merawat stoma, cara mengganti tali kanul, cara pemberian diit melalui sonde/NGT, ajarkan keluarga untuk memandikan dan fisioterapi dada. Materi edukasi pasien trakeostomi persiapan homecare bisa bertambah tergantung pada diagnosa atau penyakit pasien misalnya, pasien DM diajarkan cara periksa mandiri GDS dengan alat.

Trakeostomi di Masa Pandemi Coronavirus

Pasien covid 19 cenderung membutuhkan dukungan oksigenasi yang kontinyu dan berlangsung dalam jangka waktu cukup lama. Perburukan kondisi pernafasan mungkin saja terjadi sehingga membutuhkan ventilasi mekanis. Pasien covid 19 dengan ventilasi mekanis perlu penanganan secara khusus untuk menghindari paparan terhadap petugas kesehatan. Trakeostomi dini saat tes swab PCR positif mungkin dapat meningkatkan resiko transmisi virus. Kebutuhan trakeostomi pasien covid 19 perlu memperhatikan masa inkubasi dan hari mulai timbul gejala sehingga dapat dihindari resiko penularan virus. Keputusan late-trakeostomi lebih disukai karena menghindari resiko transmisi yang tinggi serta pasien covid 19 dengan ventilasi mekanis lebih sulit di weaning dalam waktu dekat.
